Birth Registration

(If Child is Younger Than One Year)

1. Notification of Birth Form
2. Yellow Health Card, or proof of birth (If lost or not born in a hospital, please see FAQs)
3. Marriage Certificate if parents are married
4. Parent’s certified Birth Certificate, ID or Passport (for each parent wishing to be on the child’s Birth Certificate);
5. Parent’s Passport (for each non-Namibian parent wishing to be on the child’s Birth Certificate)
6. Namibian Immigration Status (Permit, Citizenship Certificate or Visa, for each non-Namibian parent wishing to be on the child’s Birth Certificate)
7. Affidavit giving parental consent for each parent not present, but wishing to be named on the Birth Certificate

N$ 100 - Incremental for every duplicate issued i.e
1st duplicate = N$100.00
2nd duplicate = N$200.00
3rd duplicate = N$300.00
4th duplicate = N$400.00
5th duplicate and other subsequent duplicates = N$500.00

(Insertion of father's particulars for persons 6 years old and above)

5. Affidavit (from each parent involved) stating parental consent for change
