  2. Medical report from doctor who certified the death
  3. Police report (if the death occured outside of a hospital)
  4. Original ID, Full Birth Certificate or Passport of deceased and of family member registering the death

  2. Identification of deceased (Original and certified copy of Birth Certificate or ID)

  4. Affidavit setting out what happened  to the original one

N$100.00 Incremental for every duplicate issued i.e
1st duplicate = N$100.00
2nd duplicate = N$200.00
3rd duplicate = N$300.00
4th duplicate = N$400.00
5th & all other subsequent duplicates = N$500
Marriage Certificate Amendment
  1. Correction Form (signed by both parties)
  2. Marriage Certificate (Original and certified copy)
  3. Supporting documents proving the mistake on the Marriage Certificate
