1. Completed application form for passport.
2. Copy of full or abridged Namibian birth certificate; please bring along the original birth certificate for verification  purposes).
3. Copy of Namibian citizenship certificate (for citizens by descent, marriage, naturalization or registration); bring along original citizenship certificate for verification purposes .                                                                                    
4. Copy of  Namibian Identity Document (ID) or  receipt as proof that you have applied for an ID  .                                                                                         
5. Copy of SWA Namibian Identity Card and receipt as proof that you have applied for a Namibian ID.
6. Copy of marriage certificate or Divorce Order (women only).
7. One (1) passport size colour photo with clear background if applying in the regions or Namibian diplomatic missions / consulates, as well as when applying for minor and school going children under the age of 16 .
8. Number 8 on the application form must be certified by a commissioner of oath.
9. All applicants from the age of 13 years are obliged to sign in a space provided under number 7 on the reverse page of the application form.
10. All copies must be duly certified  .                                                                                                                                                                                                                                                        

NB: : Applicants should  apply in person except minors up to the age of 16 years, if he/she has no ID, in this case the parent/parents should apply for them. No third parties or agents shall be allowed to apply on behalf of anyone.

1. Frequent travellers must submit certified copies of the personal data and last stamped or endorsed page of the current passport if it is full
2. An application for a lost passport should be accompanied by a police declaration on where, when and how the passport got lost.
3. An application for a damaged passport will be approved provided that the damaged passport is attached to the application form.
4. Should an applicant wish to retain his/her current passport due to a valid visa in it, such applicant should make that request upon submission of new passport application.
